Tailored Care Solutions was born from a deeply personal experience. Our founder, Sarah Mitchell, watched her own family struggle to coordinate care for her aging grandmother across multiple healthcare providers, insurance companies, and care facilities.

The complexity was overwhelming. Important information fell through the cracks. Family members felt lost and frustrated. Sarah realized that families needed more than just healthcare services—they needed a dedicated advocate who could see the big picture and coordinate all the moving pieces.

As a Registered Nurse with over 15 years of experience in dedicated home care and Toronto's healthcare system, Sarah had the clinical knowledge and professional relationships to make a difference. She founded Tailored Care Solutions with a simple mission: to be the caring, knowledgeable advocate that every family deserves.
