Terms of Service

1. Service Description

Tailored Care Solutions provides home care consultation services in the Greater Toronto Area. Our services include:

• Assessment of care needs
• Caregiver placement and matching
• Care coordination and oversight
• Family consultation and support
• Quality assurance and monitoring

We are consultants, not direct care providers. All physical care services are provided by licensed, independent care professionals.

2. Eligibility and Acceptance

Our services are available to:

• Residents of Ontario, Canada
• Individuals 18 years or older
• Legal guardians or power of attorney holders
• Family members with proper authorization

We reserve the right to decline services if we cannot adequately meet your needs.

3. Consultation Process

3.1 Initial Assessment

Our consultation process includes:

• Comprehensive needs assessment
• Care plan development
• Caregiver screening and selection
• Background checks and reference verification

3.2 Ongoing Services

We provide continuous oversight including:

• Regular check-ins and monitoring
• Care plan adjustments as needed
• 24/7 support hotline
• Replacement services if needed

4. Fees and Payment

4.1 Consultation Fees

Our fees are clearly outlined before service commencement and may include:

• Initial consultation and assessment
• Care coordination and oversight
• Administrative and placement services
• 24/7 support services

4.2 Caregiver Costs

Direct care costs are separate from our consultation fees and are paid directly to caregivers or their agencies.

4.3 Payment Terms

• Consultation fees are due upon service commencement
• Monthly coordination fees are billed in advance
• Late payments may incur additional charges
• All payments are non-refundable except as required by law

5. Limitations and Disclaimers

5.1 Medical Care

We do not provide medical care or medical advice. All medical decisions should be made in consultation with qualified healthcare professionals.

5.2 Emergency Situations

In medical emergencies, contact 911 immediately. Our services are not intended for emergency medical response.

5.3 Caregiver Performance

While we carefully screen all caregivers, we cannot guarantee their performance. We will work to resolve any issues promptly.

6. Privacy and Confidentiality

We protect your personal information in accordance with:

• Personal Information Protection and Electronic Documents Act (PIPEDA)
• Personal Health Information Protection Act (PHIPA)
• Our Privacy Policy

Information is only shared with authorized care providers and as required by law.

7. Termination

7.1 By Client

You may terminate services with 48 hours written notice. Prepaid fees are non-refundable except where required by law.

7.2 By Tailored Care Solutions

We may terminate services if:

• Payment terms are not met
• Safety concerns arise
• Service requirements change beyond our capacity
• Terms of service are violated

8. Liability and Insurance

Tailored Care Solutions maintains comprehensive liability insurance. Our liability is limited to the cost of our consultation services. We are not liable for:

• Actions or negligence of individual caregivers
• Medical emergencies or health complications
• Property damage by third parties
• Consequential or indirect damages

9. Governing Law

These terms are governed by the laws of Ontario, Canada. Any disputes will be resolved through binding arbitration in Toronto, Ontario.
